LifeWell’s plans include programs offered by Communit-Y Health Network (CHN), a wellness and health management company. Many people need and want to change lifestyle habits. However, behavior change is hard. It sounds easy to say “lose 10 pounds” or “start exercising.” The reality is that, if it were easy, our country wouldn’t have the public health crisis that it has today. Everyone wants to be healthy.
Here’s how they work:
Each employee is asked to complete a CONFIDENTIAL Health Risk Assessment and provide some basic biometric data (blood pressure, basic blood work, Body Mass Index, etc.). The information gathered will not be shared with any company officials, only with the CHN Health-Y coaches. Screenings will take place at designated worksites and will be handled in a confidential manner.
After the data is analyzed, the employee receives personal information in written format. A Health-Y Coach will discuss the particular risk factors in person with each employee and together they will develop a Strategy. Based on the evaluation, the employee may be eligible for enrollment in a Risk Reduction program – at no cost. The coaches are trained to evaluate each individual’s situation to achieve maximum results. Programs include exercise training, healthy nutrition, weight management, stress reduction, cholesterol management, blood pressure management, smoking cessation, diabetes self-management and other appropriate lifestyle interventions. Participants will receive weekly, one-on-one, face-to-face coaching over the course of one or more 12-week periods.
